RELEASE AND LICENSE AGREEMENT

I hereby acknowledge that I have given permission for my photograph, name, occupation and story for possible inclusion in a (video/online material/social media/print material and/or ad) on behalf of the COVID-19 Vaccine Education and Equity Project. I have cooperated willingly and voluntarily in participating in making these materials available and hereby grant an unrestricted, non-exclusive, perpetual license to the COVID-19 Vaccine Education and Equity Project to use, reproduce, edit, modify, publish, and/or broadcast my footage, image and voice, and to use the same in any and all medium, including the Internet, without any financial compensation for granting the COVID-19 Vaccine Education and Equity Project such license. I acknowledge that the COVID-19 Vaccine Education and Equity Project, in its discretion, may choose not to use my submission.

I agree that I support the overall mission of the project to promote open dialogue, engagement and effective communication around authorized COVID-19 vaccines in the U.S.

I AGREE THAT IN NO EVENT SHALL THE COVID-19 VACCINE EDUCATION AND EQUITY PROJECT BE LIABLE FOR ANY DAMAGES WHATSOEVER ARISING OUT OF OR RELATED TO ANY PARTICIPATION BY ME IMAKING THIS RECORDING, INCLUDING, BUT NOT LIMITED TO DIRECT, INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL OR PUNITIVE DAMAGES, WHETHER UNDER A CONTRACT, TORT, OR ANY OTHER THEORY OF LIABILITY, EVEN IF RESERVOIR IS AWARE OF THE POSSIBILITY OF SUCH DAMAGES.

The terms of this Release and License Agreement (“Agreement”) shall be construed in accordance with the laws of the District of Columbia. The terms of this Agreement will be deemed to be a waiver of any other right or a waiver of the same right at any other time. This Agreement constitutes the entire agreement between the parties.
