As a member of the COVID-19 Vaccine Education and Equity Project, our organization agrees to support the overall mission of the project and commits to participating in, and amplifying activities and project-approved materials that promote open dialogue, engagement and effective communication around approved COVID-19 vaccines in the U.S. By completing this form, I certify that I am an authorized representative of the organization and agree to allow the organization’s name and logo to be used on campaign materials, including the COVID-19 Vaccine Education and Equity Project’s website, social media channels, and press releases.
